Appointments:

Bloo Dental is accepting new patients. You are welcome to call our office to schedule an appointment. We request 48-hours notice if you need to cancel your appointment.

Forms and medical records:

Once you have scheduled your first appointment with Bloo Dental, we invite you to securely complete our online new patient information form and medical & dental history form. Please, provide all the information requested to the best of your ability. Do not select the submit button until you have completed all five (5) pages of the patient information form and all four (4) pages of the medical & dental history form.

In addition to completing your forms, it is very useful to bring any prior dental records with you to your appointment. Your prior dental records will help the dentist and staff better serve you, and, if your record includes current x-rays, it may not be immediately necessary to take new ones.
